Community Activities Specialist

- Consult & plan community activities, which allow individuals to utilize community resources and develop natural supports.
- Provide transportation for clients to various activities and appointments through the use of agency or own vehicle
- Provide training & guidance in the areas of activities of daily living, community integration, and economic self-sufficiency skills
- Provide direct personal care or oversight as needed
- Ensure medication needs are met by monitoring and documenting medication regimen.
- Must have HS diploma/GED, be over 18 years old

- Experience working in a residential setting with individuals with developmental disabilities is preferred.
Direct Support Professional / Community Integrated Living Specialist - PRN

Family Educator

Team Leader / House Manager- New Trenton home

Director of Nursing

- Establish & maintain policies to improve the standard of care for patients
- Oversee & coordinate daily operations of the nursing department by assessing & reporting progress in meeting department objectives
- Manages department staffing needs as well as on call nursing schedule and participates in the rotation.

Mental Health Consultant Early Head Start
- Part-time 18 hours a week
- Corking with individuals & families from a variety of ethnic cultures, socioeconomic backgrounds, family compositions, etc.
- Conduct home visits with low-income families in Clinton & Washington counties (Due to COVID-19, visits are currently being conducted virtually.)
- Collaborate with staff & community providers to provide comprehensive services to enrolled families.
- Administer social-emotional screenings to all enrolled children, and the Edinburgh Postnatal Depression Screening to all expectant mothers enrolled in the program.
- Provide consultation to enrolled families when requested.
- Document interaction with families
- Attend meetings, conferences, and workshops as necessary
- Must be credentialed as one of the following: LSW (Licensed Social Worker), LCSW (Licensed Clinical Social Worker), LPC (Licensed Professional Counselor), or LCPC (Licensed Clinical Professional Counselor)
- Must have a valid driver's license and auto insurance

QIDP/Program Manager Home Based
- Full time position with benefits
- Provides support and advocacy to a group of participants and their families.
- Develops written plans based upon choices, preference, needs & desires of individual & their family
- Reports on individuals progress to supervisor, participant, guardians/families as required
- Engage in person centered philosophy and collaborative approach.
- Required: Bachelor's degree in a human service field, including, but not limited to psychology, special education, or rehabilitation counseling
- Required: 1 year experience working directly with individuals with Intellectual or other Developmental Disabilities.
