  • Duties include: assisting the individuals with all aspects of home living; cooking, cleaning, personal development, recreational activities
  • Must have a High School diploma/GED, be at least 18 years old & have a valid driver's license
  • Prefer experience as DSP/CNA, but will provide paid training

Community Activities Specialist

  • Part-time position 
  • Consult & plan community activities, which allow individuals to utilize community resources and develop natural supports.
  • Provide transportation for clients to various activities and appointments through the use of agency or own vehicle
  • Provide training & guidance in the areas of activities of daily living, community integration, and economic self-sufficiency skills
  • Provide direct personal care or oversight as needed
  • Ensure medication needs are met by monitoring and documenting medication regimen.
  • Must have HS diploma/GED, be over 18 years old
  • Must have valid driver's license &  clean driving history
  • Experience working in a residential setting with individuals with developmental disabilities is preferred.  

 

Direct Support Professional / Community Integrated Living Specialist - PRN

  • Part-time/PRN positions (as needed) with competitive wages (days, evenings, overnights)
  • Seeking a dependable & motivated staff to work with adults with developmental disabilities in classroom, production and/or residential settings
  • Duties include: assisting clients with daily living skills and supporting individuals in various activities in their daily lives ( work setting, classroom setting, residential setting)

 Family Educator

  • Full time with benefits
  • Flexible hours depending on needs of the home
  • Provide community based early education to parents with infants & toddlers as well as pregnant mothers
  • Completes developmental, behavioral & sensory screenings for all children on caseload and addresses concerns when necessary
  • Provides prenatal education to expectant women & families
  • Uses Parents-As-Teacher curriculum to work with parents & guardians to educate children in the home
  • Conducts weekly home visits with low-income families in Clinton & Washington Counties. (Due to Covid-19 visits are conducted virtually.)
  • 1 year experience working with children birth to age 5
  • Required CDA (Child Development Associate) or equivalent coursework as part of an Associate's degree, or Bachelor's degree in Early Childhood Education or Child Development
  • Must have a valid driver's license & auto insurance
  • Bilingual English/Spanish preferred, not required

Team Leader / House Manager- New Trenton home

  • Schedule & supervise staff for the residential home
  • Provide, oversight & training for staff in the home
  • Plan, organize, & direct activities for the operation of the home
  • Monitor & assist in medication administration
  • Instruct & assist participants in activities of daily living
  • Ensure learning, skill development & direction towards the most independence possible for clients
  • Bachelor's degree in human service field, or minimum 2 years experience/training related to caring for individuals with developmental disabilities or other special need populations

Director of Nursing

  • Full time salaried with benefits
  • Establish & maintain policies to improve the standard of care for patients
  • Oversee & coordinate daily operations of the nursing department by assessing & reporting progress in meeting department objectives
  • Manages department staffing needs as well as on call nursing schedule and participates in the rotation.
  • Addresses various health & medication needs on a daily basis
  • Reports on individual's health & wellness to team as well as guardians and/or families as required
  • Establishes constructive working relationships with individuals, managers, guardians/families in order to ensure health needs of the individuals are met.

  • Current IL RN license
  • 2 years clinical experience within the last 5 years
  • 1 year experience working directly with individuals with Developmental Disabilities preferred

  Mental Health Consultant Early Head Start

  • Part-time 18 hours a week
  • Corking with individuals & families from a variety of ethnic cultures, socioeconomic backgrounds, family compositions, etc.
  • Conduct home visits with low-income families in Clinton & Washington counties (Due to COVID-19, visits are currently being conducted virtually.)
  • Collaborate with staff & community providers to provide comprehensive services to enrolled families.
  • Administer social-emotional screenings to all enrolled children, and the Edinburgh Postnatal Depression Screening to all expectant mothers enrolled in the program.
  • Provide consultation to enrolled families when requested.
  • Document interaction with families
  • Attend meetings, conferences, and workshops as necessary
  • Must be credentialed as one of the following: LSW (Licensed Social Worker), LCSW (Licensed Clinical Social Worker), LPC (Licensed Professional Counselor), or LCPC (Licensed Clinical Professional Counselor)
  • Must have a valid driver's license and auto insurance
  • Proficient at using technology

QIDP/Program Manager Home Based

  •  Full time position with benefits
  • Provides support and advocacy to a group of participants and their families.
  • Develops written plans based upon choices, preference, needs & desires of individual & their family
  • Reports on individuals progress to supervisor, participant, guardians/families as required
  • Engage in person centered philosophy and collaborative approach.
  • Required: Bachelor's degree in a human service field, including, but not limited to psychology, special education, or rehabilitation counseling 
  • Required: 1 year experience working directly with individuals with Intellectual or other Developmental Disabilities.
  • Required: valid driver's license and auto insurance

 

 

Benefits for Full Time Employees:

Holidays - 12 paid holidays per year

Paid vacation & sick leave -  10 days vacation  & 6 sick days first year  (incremental increases in both based on years of service)
